Question 49
Dear Preschool Teacher,
I am in the process of starting up my own
preschool. What forms do I need for the parents to fill out
(registration, medical, etc.)? Where can I find forms like this
or do I make them up myself?

Forms,
I made up the forms that we use at my preschool. My preschool admission form has parent information, work information, telephone numbers, people to contact in case you can't contact the parents. A place for personal information such as siblings, pets in the family, any allergies, etc. On the back of the preschool admission form, I have a liabilities release form where the parents have to sign to say it's o.k. for us to take them on field trips. In case of an emergency, there is a place to put the Drs. phone number and what hospital they wish their child to be transported to.
As far as medical forms go, I get them from the agency that regulates licensing for my state.

You could start out by searching online for information. There are so many
websites that were created by people who run their own daycare's and have
made available their paperwork and information for others who are just
starting up. If you are a member of any of the egroup.com childcare loops, just
check the files and you could find some there.

Dear Preschool Forms,
I would think your licensing agency would supply you with the forms that
are required. Other than that, ask other schools or daycare what they use.
You will want personal information also, so maybe check out some preschool /
daycare sites for information.

The forms we use at our center come from the state. Other than our registration form, which we designed ourselves.
